Most chiropractic and physio clinics don't have a new-patient problem. They have a drop-off problem: patients feel better after a few sessions and quietly stop booking long before their care plan is finished. These are the GoHighLevel automations I built to catch that, with the results, the messages and what stays with the practitioner.
The results first
- 42% more patients completing their full treatment plan
- 15 at-risk patients a day flagged automatically before they drop off
- Under 3 minutes from online booking to confirmed appointment
- 23% more revenue per patient episode from better plan completion

1. Fast replies to new-patient enquiries
New patients are often in pain and ring several clinics. Missed calls get an instant text, and web or ad enquiries get a reply within a minute with a booking link for a first assessment. See missed-call text-back.
2. Online booking and intake
Patients book their first assessment from live availability, matched to the practitioner and appointment type, and fill in contact and admin details online. Keep clinical history in your practice software or a HIPAA-covered form (see the privacy section).
3. Reminders at 48 hours and 2 hours
A confirmation when booked, then reminders 48 hours and 2 hours before each session, with one-tap confirm or reschedule. Two reminders work better than one for regular treatment visits.
4. Track the care plan, not just the next appointment
This is the core of the build. After the assessment, the practitioner records the plan on the patient's record with a few custom fields:
- Visits recommended (for example 8)
- Visits completed (updated after each session)
- Recommended interval (for example weekly)
- Next visit due (calculated from the last visit)
The practitioner still decides the plan and changes it as the patient progresses. The system just keeps track, so nobody has to.
5. Rebooking before the patient drifts
If a patient leaves without booking the next session, a short reminder goes out around the time their next visit is due:
Hi {{contact.first_name}}, it's {{business_name}}. Your next visit is
due around {{contact.next_visit_due}} and you don't have one booked yet.
Grab a time that suits you: {{booking_link}}6. The drop-off alert
When a patient on an active plan passes their due date with nothing booked, the system flags them for the front desk and sends a friendly check-in, without clinical detail:
Hi {{contact.first_name}}, we noticed you haven't booked your next
session. How are you feeling? If anything's changed or the timing is hard,
reply and we'll help find a time that works.A reply goes straight to reception. No reply after a few days creates a call task. This is where the 42% improvement in plan completion came from: catching drop-off within days, not months.
7. Reactivation for lapsed patients
Patients who haven't visited in 3–6 months (or longer) get a reactivation sequence:
Hi {{contact.first_name}}, it's been a while since your last visit
to {{business_name}}. If the old problem is creeping back, or you'd like a
check-up, we have times this week: {{booking_link}}The same approach works for any clinic; see automated patient recall.
8. Reviews at the right moment
Ask for a review when the patient completes their plan, when they feel the improvement most, not after the first visit. See Google review automation. Maintenance or wellness plans (a monthly visit, billed as a membership) are a natural next offer at the same point.
Privacy: HIPAA and patient messages
- US clinics: GoHighLevel needs HighLevel's HIPAA add-on with a signed BAA for patient data. See is GoHighLevel HIPAA compliant?
- Keep conditions and treatment details out of texts and emails. "Your next session", not the injury.
- Texting: US clinics need A2P 10DLC registration; UK and EU clinics should follow GDPR and PECR.
Connecting your practice software
Your practice management software (ChiroTouch, Jane, Cliniko, Nookal and others) stays the record for notes and billing. To keep GoHighLevel in step, use the software's API where it has one (Cliniko does, for example), Zapier or a sync tool, or scheduled exports of appointments and last-visit dates. See connecting any app to GoHighLevel.
